POSITION TITLE: Physician or Nurse Practitioner- In-Home Primary Care
RESPONSIBLE TO: Executive Director
POSITION SUMMARY:
The Physician and/or Nurse Practitioner providing in-home primary care services is a vital member of the healthcare team, responsible for delivering comprehensive and compassionate medical care to patients in their homes. This role involves the assessment, diagnosis, and management of various health conditions for patients who may be homebound or prefer receiving care in the comfort of their own living environments. The Physician or Nurse Practitioner will work collaboratively with patients, families, and other healthcare professionals to ensure high-quality, patient-centered care that meets the diverse needs of the community.
ESSENTIAL DUTIES AND RESPONSIBLITIES:
Conduct thorough in-home evaluations, including physical examinations, health history reviews, and psychosocial assessments to determine patient needs and health status.
Diagnose acute and chronic illnesses and develop appropriate treatment plans based on evidence-based guidelines and individual patient needs.
Collaborate with other healthcare providers, specialists, and community resources to facilitate comprehensive patient care and follow-up services.
Prescribe medications, monitor therapeutic responses, and educate patients on proper medication use and potential side effects.
Provide clear and compassionate education to patients and their families regarding health maintenance, disease prevention, and self-management strategies.
Maintain accurate and timely electronic medical records (EMR) in compliance with organizational policies and regulatory standards.
Participate in quality assurance activities, clinical audits, and continuous improvement initiatives to enhance service delivery and patient outcomes.
Stay current with medical advancements, best practices, and regulatory changes in primary care and home health services through ongoing education and training.
Demonstrate sensitivity to cultural, social, and economic factors affecting patient care and engage in practices that promote health equity.
QUALIFICATIONS:
Practicing physician, currently licensed under the provisions of the Oregon State Board of Medical Examiners or Nurse Practitioner (NP) in the state of Oregon.
Certification: Board certification in a relevant specialty (for Physicians) or Nurse Practitioner certification from a recognized accrediting body.
Experience: Clinical experience in primary care, home health, or a similar healthcare setting, demonstrating a strong foundation in patient care preferred.
Clinical Skills: Proficient in the assessment, diagnosis, and management of a wide range of medical conditions, with a focus on preventive care and chronic disease management.
Communication Skills: Excellent interpersonal and communication abilities, with a strong commitment to patient-centered care and collaborative practice.
Technology Proficiency: Familiarity with electronic medical record (EMR) systems and telehealth technologies.
Transportation: Valid driver's license and reliable transportation to travel to patient homes as needed.
WORK ENVIRONMENT:
Travel to patients' homes across designated service areas in Central Oregon, and flexibility to adapt to various home environments and other varying condition
The role may also include occasional telehealth services, depending on patient needs and organizational policies.
