Description: Mission to rebuild healthcare as it should be", providing personalized primary care for older adults on Medicare, with the goal of keeping patients healthy and living life to the fullest. Our innovative care model is centered right in our patients communities and focused on the quality of care over the volume of services.- Salary: Range is 105,000-135,000 (Experienced provider with bonus can make $150-$160k)
- Annual clinical performance bonus (last year NP's with less than 5 years experience made 15k annually and more seasoned with 5+ years exp. made 25k bonus)
- Reimbursement for Mileage and Toll fees
Rate: $105000-$160000
Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- Ideally 5+ years' experience in either primary care in clinic or Home setting with geriatric population
- Active, non-probationary Pennsylvania Nurse Practitioner license
- Active DEA license
- US Work Authorization
- Post discharge visits/ transitional care experience a plusNational certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
Responsibilities:
Role: Transitional Health - (Home Health)
- Focused on Post Discharge visits Includes -Medication reconciliation -Social service follow ups
- Follow up care in general with follow up care coordination.
- Seeing 6-7 patients a dayWill be supported by the Medical Director
